Athletics

Kaleidoscope Academy is proud to offer many co-curricular options. Athletics start for our 7th and 8th graders, while other clubs and extracurricular options are available for all students.

Athletics are a great way to connect with classmates and get involved with the Kaleidoscope community. Being part of a team also helps develop lifelong skills such as teamwork and time management.

Many athletic teams and clubs meet after school. To accommodate family and religious events, all activities end before 5:30 p.m. on Wednesday evenings.

To ensure your student can participate, please follow the three-step registration process outlined below.

Step 1: Complete the Sport-Specific Sign-Up Form

Most of the paperwork previously completed with paper and pencil will now be done electronically. Each season has a unique form that needs to be filled out to indicate your student’s interest in participating.

In each sports section below, you'll find a link to the online sign-up form, along with schedules, coach contact info, and more. This form must be filled out if your student will be participating in that sport at KA during the school year.

This online paperwork must be filled out for each student-athlete on a yearly basis in order to participate. If you have multiple children participating, you will need to fill out one form for each.

Step 2: Fill Out the Middle-Level Athletics Agreement Form

The next step is to fill out the Middle-Level Athletics Agreement Form. This form includes:

Review and acknowledgment of:

  • Concussion and Sudden Cardiac Arrest information
  • Code of Conduct agreement
  • Liability Waiver
  • Alternate Year Form (required in the second year of a physical)

Step 3: Submit the Physical Form

A WIAA Physical Form signed and dated by a medical provider is required before a student may try out, practice, or compete.

  • Physicals are valid for two years, but yearly updates are strongly encouraged
  • Forms must be turned in to the main office or faxed to (920) 852-5431
  • Physical forms are available in English, Spanish, and Hmong
  • Copies are available at medical offices or through the school office

STEP 4: Pay the Participation Fee

  • $40 participation fee per sport
  • Payment must be turned in to the office before each season begins
  • Payment can be made by cash, check (Written to Kaleidoscope), or via Infinite Campus (instructions can be found here).
  • Free and Reduced: If your family is free and reduced, fees are automatically waived

Note on Athletic Fees

  • Per Activity: $40.00
  • Student Yearly Cap: $120.00
  • Family Cap: $160.00 (for multiple students in the same family)
  • Joint Middle and High School Family Cap: Maximum: $250.00 (for families with students in both middle and high school)

Important Notes

  • Students may not participate in tryouts, practices, or games until all steps above are completed.
